If you are interested in establishing care with me, please take the time to complete all intake forms. All new patients or coaching clients MUST provide a fully completed set of paperwork for our first visit. If they are not done by your scheduled appointment time you may be asked to reschedule.

Although lengthy, you are encouraged to take the time to fill out these forms in detail. This information allows me to have a bit of your history so that we can focus our initial visit on your current health concerns and goals, while treating you holistically and uniquely. My desire is to always use our time together most efficiently for your greatest progress!
For in-person visits, if you are unable to complete intake forms prior to your appointment, please plan to arrive a minimum of 30 minutes early.
For virtual consultations, we will need to reschedule if I do not receive your forms at least 24 hours ahead of your appointment. You may send them as instructed above.
In addition; for Naturopathic appointments and Wellness coaching, please bring or share copies of any labs you have had done in the past 2 years via fax, dropbox or google drive.
For Auto accident claims, please fill out both the MVA form and sign all Consent forms.
You will have an opportunity to ask any questions you have about the content of the “Consent to Treat” at your visit if you need.

If you are a resident of Oregon many insurance companies provide alternative care benefits that cover Naturopathic and Acupuncture services. It is encouraged that you verify your benefits as not all policies have this coverage. If your policy covers my services, my office staff is happy to submit your claim directly. Please consult your preferred provider list, or call the office with your insurance information to see if I participate in your plan.
Currently I am an in-network provider for Regence-Blue Cross Blue Shield (Blue card, Local, Federal), Moda, Pacific Source, Great-West, and all Motor Vehicle Insurance.
If your insurance is not on this list, please note that plans often have out-of–network benefits and we are able to submit your claim in that case. I offer a generous “at time of service discount” for those who do not have insurance coverage. If you desire to work with me, we can always find a solution.
We are happy to verify your insurance eligibility and bill for your treatments if you have coverage. Please see our office policies included in your new patient paperwork regarding this. We cannot guarantee benefits, as they must be current at the time of service and deductible met for coverage. We ask that you inform us of any changes to your plan.
Please note that I am unable to bill insurance for phone or video consultations.
Please educate yourself about your coverage, you will be responsible for any services rendered not covered by your plan.
It is beneficial to remember that everyone involved in a motor vehicle accident in the state of Oregon is entitled to $15,000 of medical care including acupuncture. Oregon personal injury protection (PIP) covers acupuncture care no matter who was at fault. Acupuncture can be an effective and restorative treatment to realign your body, reduce pain and inflammation and speed your recovery. Do not wait to rebalance your system after such an experience. No referral is required and I can refer you for massage if it is reasonable and necessary.
My rates are set at community standards for in-clinic consults and acupuncture.
Payment is expected at time of service.
